Parents registering their child
will be required to provide the following at
the time of registration:
• Proof of residence (full page reflecting service address and name) dated in the last 30 days in the form of :
* electric or gas bill
* water bill
* household property tax (not car taxes)
* current rental agreement (valid for 30 days and then one of the other forms must be submitted)
* phone bill (land line only) - cellular phone bills will not be accepted
* cable bill
• Immunization record - new students only
• Birth certificate - new students only
Please note that registration for our 4K program also requires a Medicaid card or proof of income. There are a limited number of spots available. Once filled, students may be placed on a waiting list.
